Transformation Plans Terms & Conditions
By purchasing a Skin Sanctuary Six-Month Transformation Plan, you acknowledge that you have read, understand, and agree to the following terms and conditions.
1. Plan Options:
Six-Month Transformation Plan — $1,499
Includes:
• 1 Strategy Facial
• 3 Microneedling Treatments
• 1 DiamondGlow Treatment
• 1 Final Strategy Facial
Advanced Six-Month Transformation Plan — $2,499
Includes:
• 1 Strategy Facial
• 1 Microneedling Treatment
• 1 SkinVive Treatment
• 1 Additional Microneedling Treatment
• 1 DiamondGlow Treatment
• 1 Final Strategy Facial
• 20 units of Botox as a promotional bonus, when medically appropriate
Treatment timing and sequencing may be adjusted by your Skin Sanctuary provider based on your individual skin needs, treatment response, medical history, scheduling, and clinical appropriateness.
2. Payment:
Transformation Plans are prepaid at the time of purchase unless otherwise specifically arranged by Skin Sanctuary. Payment secures your plan and the promotional pricing associated with it. Individual services within a Transformation Plan may not be separated, exchanged for cash, or individually refunded based on their regular retail value.
3. Plan Expiration
Transformation Plans are designed to be completed over approximately six months. All included services must be completed within 12 months of the original purchase date, unless Skin Sanctuary approves an extension due to a documented medical reason or other circumstance at its discretion. Any unused promotional or bonus services remaining after the expiration date may be forfeited. Clients are responsible for scheduling appointments with enough time to complete the plan before expiration.
4. Scheduling
Purchasing a Transformation Plan does not automatically reserve appointment times. Appointments are subject to provider and clinic availability. We strongly recommend scheduling your planned treatment series in advance, especially if you are preparing for a wedding, event, or other deadline. Skin Sanctuary cannot guarantee completion of the full treatment series before a specific event if appointments are not scheduled with adequate notice.
5. Cancellation and No-Show Policy
All Transformation Plan appointments remain subject to Skin Sanctuary’s standard cancellation and no-show policy. Late cancellations, missed appointments, or failure to follow the cancellation policy may result in applicable fees.Repeated missed appointments or late cancellations may impact the ability to complete the plan within the designated expiration period.
6. Medical Eligibility
Some treatments included in these plans are medical aesthetic procedures and require appropriate medical screening and provider approval. Purchase of a Transformation Plan does not guarantee that every included procedure will be medically appropriate for every client. Treatment may be postponed, modified, substituted, or declined based on:
• Pregnancy or breastfeeding
• Current medications
• Medical conditions
• Skin conditions or active infections
• Recent procedures
• Allergies
• Treatment response
• Provider assessment
• Contraindications
• Other safety concerns
• Clinical safety will always take priority over the original treatment schedule.
7. Treatment Modifications
Your provider may recommend adjusting the timing, order, or type of treatment included in your plan if doing so is more appropriate for your skin or medical needs.
When possible, Skin Sanctuary will work with you to substitute an appropriate treatment of reasonably comparable value if a planned service becomes medically inappropriate. Treatment substitutions are determined by Skin Sanctuary and are not guaranteed to be available in every circumstance.
8. SkinVive Eligibility
SkinVive is an injectable treatment and requires medical evaluation and informed consent.Clients must be appropriate candidates before treatment. If SkinVive is determined to be medically inappropriate, Skin Sanctuary will review alternative options with the client.
9.**MEMBERS** Botox Promotional Bonus
The 20 units of Botox included with the Advanced Transformation Plan are a promotional bonus and have no cash value. Botox treatment requires medical evaluation and must be deemed clinically appropriate.
The promotional Botox:
• Is limited to a maximum of 20 units
• Cannot be exchanged for cash or retail credit
• Cannot be transferred to another person
• Cannot be applied toward another injectable product
• Must be used within the plan’s expiration period
• May not be combined with other Botox promotions unless specifically approved by Skin Sanctuary
• If Botox is medically contraindicated, Skin Sanctuary will determine whether an alternative promotional option is available.
Every client’s skin, anatomy, healing response, and treatment response are different. Purchasing a Transformation Plan does not guarantee a specific cosmetic outcome. Results may vary depending on factors including age, genetics, lifestyle, skincare compliance, sun exposure, health, treatment tolerance, and adherence to recommended aftercare.
11. Weddings and Special Events
Transformation Plans may be used as part of a bridal or event-preparation strategy; however, Skin Sanctuary does not guarantee a specific result by a particular date.Treatment timing may need to change depending on your skin response, healing, medical eligibility, or appointment availability. Clients should communicate all major event dates to their provider before beginning treatment.
12. Refunds
Transformation Plans are non-refundable.
13. Promotional Pricing
Transformation Plan pricing is promotional and may be changed or discontinued at any time for future purchases. Once purchased, the client will receive the services included in the plan according to the terms in effect at the time of purchase.
14. Separate Medical Consent
Acceptance of these Terms & Conditions does not replace informed consent for individual medical or aesthetic procedures. Clients will be required to complete any applicable medical history forms, treatment-specific consents, photography consents, or other clinical documentationr equired by Skin Sanctuary before treatment.
15. Agreement
By completing your purchase, you confirm that:
• You understand which services are included in your selected Transformation Plan.
• You understand that treatments must be completed within the designated plan period.
• You understand that treatments are subject to provider assessment and medical eligibility.
• You understand Skin Sanctuary’s scheduling, cancellation, refund, and expiration policies.
• You understand that individual results cannot be guaranteed.
• You agree to these Six-Month Transformation Plan Terms & Conditions.
